3. You must provide identification proving your identity. If ordering remotely, whether online or by mail, a notarized statement is also required. Without providing accurate information as it appears on the original certificate, a vital record will not be released. If information is unknown, most of the time it can be found by searching ancestral websites such as Ancestry.com.
